Malignant cerebral edema (MCE) is often associated with severe physical disability and a high mortality rate. The current prediction of MCE focused on infarct volume, tools are relatively lacking. prominent veins sign (PVS-SWI) considered marker severely impaired tissue perfusion. This study aimed to determine whether PVS-SWI early-onset MCE. Patients acute ischemic stroke (AIS) due large arterial stenosis or occlusion (SLASO) from June 2018 2020 were included. ASPECTS score assessed the extent PVS-SWI, 4–10 was defined as positive group. primary outcome MCE, deterioration neurological function midline structural excursions >5 mm during hospitalization. secondary outcomes included worsening NIHSS by ≥ 2 points, in-hospital death, death within 1 year after stroke. Logistic regression used assess correlation between outcomes. 157 patients, 40 (25.5%) whom developed more prevalent in patients who (75.0% vs 45.3%; P = 0.001). In multivariate analysis, an independent predictor development larger sizes (OR: 4.00, 95%CI: 1.54–10.35,p 0.004). small sizes, ≥2(OR: 11.13, 2.26–54.89, p 0.003). However, not death. main finding our that increased risk developing these patients, interventions may be needed.
